Guideliness In the Implementation of Free
Medical Care Including Subsistence, Medicines and Medical Assistance to
Barangay Officials From Philippine Heart Center (PHC).
I. Who are covered?
1. Sangguniang Barangay
i. Punong
Barangay (Barangay Chairman)
ii. Barangay Kagawads
iii. Sangguniang Kabataan Chairman
2. Barangay Treasurer
3. Barangay Secretary
II. What are the Surgical Heart Diseases covered?
1. Rheumatic Heart Disease (RHD)
2. Coronary Artery Disease (CAD)
3. Atrial Septal Defect (ASD)
4. Ventricular Septal Defect (VSD)
5. Complex Congenital Heart Disease
i.
Tetralogy of the Fallot
6. LA Myxoma
7. Ruptured Coronary Sinus of Vasalva
Note: Brain strokes are not
included
III. What is Free?
1. Room Accommodation (Service Ward)
i.
Admission shall be into any service bed at a first come first- served
basis
ii.Barangay Officials admitted at service
wards do not need Social Service clearance
2. Drugs and Medicines
3. Diagnostic and Therapeutic Procedures
4. Surgery/Surgical Expenses
5. Operating Room
6. Central Supplies
7. Professional Services (They shall be under the
care of the respective consultant-on- duty and his team)
Note: PhilHealth and Insurance
benefits shall be claimed by the Heart Center.
IV. What is Not Free?
1. STENT
i. If there is a need for a stent, the barangay
official shall procure his own stent, which costs P150,000.00.
2. VALVE
i. For Double Valve Replacement, only one Valve
shall be provided by the Center and the other shall be borne by the patient
at cost in the current market price.
3. BLOOD
i. They shall be required to provide their own Blood
Donors and in Emergency Cases, shall have to replace the blood used. The
fees for Blood Screening shall be charged to the patient accordingly.
4. MATERIALS AND SUPPLIES FOR CORONARY
ANGIOGRAM
i. Barangay Officials due for surgery may undergo
Coronary Angiogram. However, cost of consigned materials and supplies
(if needed) shall be borne by the patient.
5. UPGRADED ROOMS
i. No upgrading in Room Accommodation shall be permitted.
Should the patient opted accommodations other than the Service Ward; he
shall lose those free benefits. However, he may avail of the ten percent
(10%) government discount per existing hospital policies.
6. OUT-PATIENT SERVICES
i. If the Barangay Official is referred for out-patient
laboratory procedures, only the ten percent (10%) discount for government
employees shall be applied in accordance with the existing hospital policies.
This includes Emergency Room services.
ii. The Barangay
Official may have the option to choose his/her own attending physician
at his own cost/expense.
V. How does one avail of the Free Medical Care?
1. Consult a Doctor/Cardiologist from
the Government Hospital nearest him.
2. Secure a Medical Certificate from
the said Government Hospital.
3. Request for a Certification from the
Liga Provincial/City Chapter President and/or the DILG Provincial/City
Director stating that he is a duly elected and incumbent Barangay Official.
4. Submit all documents to the Liga ng
mga Barangay National Office.
5. Secure referral from the National
Liga Office duly signed by the National President.
6. Proceed to the Chief Fellow of the
Philippine Heart Center Adult Cardiology Unit for medical evaluation.
He shall be further assessed for management as an out or in-patient and
the Chief fellow shall order confinement at the Heart Center if the official
is suffering from any of the ailments mentioned above.
Other Requirements
Any form of Identification with Picture, preferably Liga
ng mga Barangay ID.

Guideliness

The National Kidney And Transplant Institute (NKTI).
III. WHAT IS FREE?
Officials admitted for kidney transplant shall be given
free confinement on the following for the first seven (7) days:
1. ROOM ACCOMMODATION (SERVICE WARD):
i.
Admission shall be into any service bed at a first-come first- served
basis;
ii. Barangay Officials admitted at service wards
do not need Social Service Clearance;
2. DRUGS AND MEDICINES;
3. DIAGNOSTIC AND THERAPEUTIC PROCEDURES;
4. SURGERY/SURGICAL EXPENSES;
5. OPERATING ROOM;
6. CENTRAL SUPPLIES;
7. PROFESSIONAL SERVICES (They shall be under the
care /charge of the respective consultant-on-duty and his team.)
NOTE:
Philhealth and other insurance benefits (i.e. HMO’s) shall be paid
to National Kidney and Transplant Institute
IV. WHAT IS NOT FREE?
1. COST OF HOSPITALIZATION OF DONOR:
The cost of the hospitalization of the donor shall
be paid in full by the Barangay Official;
2. BLOOD
Patient shall be required to provide their own blood
donors and in emergency cases, shall have to replace the blood used. The
fees for blood screening shall be charged to the patient accordingly.
3. OUT-PATIENT SERVICES
a. If the Barangay Official is referred for outpatient
laboratory procedures, only the twenty percent (20%) discount for government
employees shall be applied in accordance with the existing hospital policies.
This includes Emergency Room services;
b. The cost of donor work-up shall be paid in full by the official;
c. The Barangay Official may have the option to choose his/her own attending
physician at his/her own cost/expense.
4. UPGRADED ROOMS
No upgrading in room accommodation shall be permitted.
Should the patient opt for accommodations other than the Service Ward,
he shall lose these free benefits. However, he may avail of the twenty
percent (20%) government employees discount per existing hospital policies.
V. HOW DOES ONE AVAIL OF THE FREE MEDICAL CARE?
1. Consult a Doctor at a Government Hospital nearest
him.
2. Secure a Medical Certificate and a Referral from the said government
hospital
3. Secure a Certification from the Liga Provincial/City Chapter President
and/or the DILG Provincial/City Director stating that he is a duly elected
and incumbent Barangay Official.
4. Submit all documents to the Liga ng mga Barangay National Office together
with any form of Identification with Picture, preferably Liga ng mga Barangay
ID.
5. Secure Referral from the National Liga Office duly signed by the National
President.
6. Proceed to the Chief Fellow of the NKTI Nephrology Unit for medical
evaluation. He shall be further assessed or managed as an out or in-patient
and the Chief Fellow shall order confinement at the NKTI, if the official
needs kidney transplant. He shall likewise make referral to another hospital
for care if the illness is non-renal disease or if a renal disease can
be treated in a general hospital.
